Everyone’s health, home, and routine are different. Our approach begins with listening, understanding what matters to you, what support you need, and how you prefer your care to be delivered.
Once we receive a valid referral, a registered nurse will assess your needs in your home and develop a written care plan with you. With your consent, your family, carer and health professionals can be involved too.

WHO MAY BE ELIGIBLE?

Support for Eligible DVA Veteran Card Holders

You may be able to receive DVA-funded Community Nursing if you have an assessed clinical need for nursing or personal care at home and hold:

Veteran Card – All Conditions

Often referred to as a Gold Card

Veteran Card – Specific Conditions

Where care relates to an accepted service-related condition

DVA-funded Community Nursing is provided at no cost to eligible clients. For Veteran Card – Specific Conditions holders, the required care must relate to an accepted service-related condition. Eligibility is confirmed before care begins.

Arrange a Written Referral

Your GP, treating hospital doctor, hospital discharge planner or a nurse practitioner specialising in community nursing can refer you.

Send it to Libra Care

The referral should outline the services required and the relevant medical conditions. Our team can help explain what is needed.

Meet Your Nurse at Home

We check the required details, then a registered nurse visits to understand your clinical nursing and personal care needs.

Refer a DVA Client with Confidence

We welcome written referrals from GPs, treating medical practitioners in hospital, hospital discharge planners and nurse practitioners specialising in community nursing.
Referrals may use the DVA D9389 form or the referrer’s own document. Please send the completed referral directly to Libra Care and include:

Can I contact Libra Care directly?
Yes. Veterans, family members and carers are welcome to contact us and talk through the process. DVA-funded Community Nursing requires a valid written referral from an authorised health professional and confirmed eligibility before care begins.
A written referral can come from a GP, treating medical practitioner in hospital, hospital discharge planner or nurse practitioner specialising in community nursing. The referral is sent directly to Libra Care—not to DVA.
A written referral is required when you are new to Community Nursing, when you have been out of care from a Community Nursing provider for more than 28 days, and at the end of each continuous 12-month period of care.
We check eligibility, contact the client to arrange a face-to-face nursing assessment, develop a care plan and provide a copy to the client. The client’s GP continues to have clinical oversight of their care.
DVA funds clinically required Community Nursing services for eligible clients. There is no cost to an eligible client for services delivered under the program.
Yes. With your consent, family members, carers or advocates can take part in your assessment, care planning and ongoing communication.
No. DVA Community Nursing is a separate Australian Government program with its own eligibility and referral requirements. Our team will help keep the pathway clear.
Yes. You can choose your provider. A new written referral is required when changing providers, and we can explain the handover process.
