YOUR PATHWAY TO CARE
Four Clear Steps.
One Supportive Team.
1
Arrange a Written Referral
Your GP, treating hospital doctor, hospital discharge planner or a nurse practitioner specialising in community nursing can refer you.
2
Send it to Libra Care
The referral should outline the services required and the relevant medical conditions. Our team can help explain what is needed.
3
Meet Your Nurse at Home
We check the required details, then a registered nurse visits to understand your clinical nursing and personal care needs.
4
Begin Your Tailored Care
We create a written care plan with you, coordinate your support and review it as your needs change.
